Team Registration/ Captain Information
By filling out this form, you are agreeing to become your team's captain. You will receive any and all notifications and schedule for your team and the 3rd Annual Love, Patti Adult Softball Tournament.
After this form is submitted, your team will be registered! After registration, players can submit their payment, shirt size and customization in the "Adult Co-Ed Player Registration" form also on this page. Payment is $50 and includes a ticket to the "Family Fun Day Festival" Players must input their team name in order to be on the correct roster in the Player Registration form.
Please note: You must also submit your own player registration form and payment if you are playing in the tournament or purchase your own ticket to the "Family Fun Day Festival "for $50. You will not receive a team shirt if you do not register as a player,…
