Team Registration/ Captain Information

By filling out this form, you are agreeing to become your teams captain. You will receive any and all notifications and schedule for your team and the 2nd Annual Love, Patti Adult Softball Tournament.

After this form is submitted, your team will be registered! After registration, players can submit their payment, shirt size and customization in the "Adult Co-Ed Player Registration" form also on this page. Payment is $75 and includes a ticket to the "Family Fun Day Festival" Players must input their team name in order to be on the correct roster in the Player Registration form.
