By submitting this form, you are registering a team and agreeing to serve as the team captain. All tournament communication, including schedules and updates, will be sent to the captain listed on this form.

Next Steps for Players: Each player must complete the Adult Co-Ed Player Registration form and submit their $75 player fee (including captain if captain is playing). This fee includes:
Players must enter the team name/ captain name on the Player Registration form in order to be placed on the correct roster.
Important for Captains: If you are playing in the tournament, you must also complete the Player Registration form and submit payment like all other players.
Captains who do not register as players will not receive a team shirt and will not have access to the Family Fun Day Festival unless a separate $50 Family Fun Day ticket is purchased.
Please remind your team to register and submit payment as soon as possible. Player registration closes June 27th. Players will not be added to rosters after this date.
